Patients' demands and expectations in Implant Dentistry are getting higher and higher nowadays. They chase even more aesthetic results with less morbidity possible and in a shorter time. Different approaches and prosthetic designs are possible when it comes to full mouth rehabilitations. In some cases, vertical bone reduction is required in order to hide the prosthetic transition in the smile line or to promote enough vertical prosthetic space. Very often patients find those FP3 prosthetic solutions unnatural, uncomfortable and difficult to clean. A perio-prostho approach with FP1 prosthetic design (no artificial gum) allows the clinician to restore the lost tissues architecture around implant-supported rehabilitations, providing the patient the desired aesthetics and comfort. It takes a more complex and precise reverse planning mastering implant 3D positioning and soft tissue grafting procedures. Provisionals are crucial as they will manage the emergence profiles all the way through the final restoration.
Precision of digital workflow provides much better results in such sensitive procedures, it enhances the aesthetic outcome and avoids some further reconstruction procedures comparatively with conventional approaches.
Not only in surgical stages but also in the prosthetic phase, the use of digital prosthetic tools will guide the clinician through a more predictable and comfortable path so that the final result is fitter and closer to what was planned from the beginning.
dr Pedro Moura - All-on-X Complications: How to Avoid Them, and How to Get Out of Them
Full-arch rehabilitation runs along a spectrum. At one end, a standard All-on-4 in adequate bone. At the other, zygomatic and pterygoid anchorage in a maxilla with almost nothing left to work with. Somewhere along that spectrum something is bound to go wrong, and the further along it you operate, the less room you have to recover.
This lecture covers that ground in four steps. First, risk assessment: reading the case before surgery and knowing where on the spectrum it actually sits, which is where most complications are decided. Second, the problems that occur most often at each level, during surgery and afterwards. Third, how to get out of them in the moment, with the decision options laid out as they present themselves rather than in hindsight. Fourth, what happens next — what the case looks like after salvage, and what the patient is actually left with.
Cases are from the author’s own practice, including the ones that did not resolve well.
dr. Ricardo Kern - NCCL Restoration and the BOPT Technique: Workflow in Periodontal Plastic Surgery
Non-carious cervical lesions (NCCLs) frequently present a complex challenge in periodontal plastic and restorative dentistry, particularly when tooth wear, gingival recession, cervical defects, and altered soft-tissue contours coexist. Successful treatment requires more than simply restoring the defect; it requires an integrated approach that considers tooth morphology, periodontal phenotype, gingival position, emergence profile, and the relationship between the restoration and the surrounding soft tissues.
This lecture will present a clinical decision-making algorithm for the management of NCCLs, focusing on when and how restorative treatment should be combined with periodontal plastic surgery. Particular emphasis will be placed on the Biologically Oriented Preparation Technique (BOPT) as a strategy to modify the cervical emergence profile and optimize the interaction between the restoration and periodontal tissues. Clinical cases will illustrate diagnosis, treatment sequencing, surgical considerations, provisionalization, and restorative principles aimed at achieving stable periodontal tissues and predictable esthetic outcomes.
The objective is to provide clinicians with a practical framework for selecting and sequencing restorative, surgical, and BOPT-based approaches according to the specific clinical situation.
